Community Short Breaks Service - Children and Young People survey

Derby City Council wants to know what you think about the Community Short Breaks service. If you answer these questions, you will help us to make the service better.

A Community Short Break is a fun activity not in your school. It could be a club, sport, or a trip. Children and young people with Special Educational Needs and Disabilities (SEND) can have fun, make friends, and learn new skills. It could be a club, sport, or a trip.

Parents and carers can have some time to rest.

Please submit your survey by midnight 19 August 2026
