This survey is about how children and young people share their views, wishes and feelings about their education, health, and care plan (EHCP).

An education, health and care plan is a legal document for children and young people who need extra support in school or college. It's really important that children and young people's views are included in their EHCP because this document should support them to achieve their hopes and wishes for the future.

The answers you tell us will be anonymous and the results from the questions will go to Derby City Council to improve Special Educational Needs and Disability (SEND) Services.
