Derby City Council is consulting on the transition planning process of our young people and their journey into adulthood.

This survey is for young people and their parent(s)/carer(s) to tell us about their experience of the Preparing for Adulthood process. Your answers will help us to understand the transition process and help us to identify how the support and service could be improved and developed within the constraints and resources available.

You can complete this survey in a number of ways

  • by taking our online survey (below)
  • download (on the right hand side of this page) or request a paper copy to fill in at home and return to us.
  • or by requesting a completing a copy of our online Easy Read pdf survey and returning it via email to letstalkderby@derby.gov.uk

    Preparing for Adulthood survey

    Derby City Council wants to understand the experiences of our young people and their journey into adulthood. 

     All young people and their parents have the right to have high expectations for their future. Young people with Special Educational Needs and Disability (SEND) may face additional barriers and challenges to achieving their goals. 

    Preparing for adulthood means identifying the support a young person will need to live as full and active an adult life as possible.  

     This will help us to know we are using our money and resources in the best and most effective way and will make sure the support you have is meeting your/your child's assessed needs. 

    Please submit your response by midnight 4 November.
